Use this checklist from the Knox County Health Department to make sure your child's daycare provider is maintaining a safe and healthy environment.

Chemical Hazards
Are all chemicals labeled according to contents?	Yes	No
Are chemicals stored away from children's reacts	Yes	No
Are chemicals stored away from foods and clothing?	Yes	No
Are all medicines stored properly? Stored in refrigerator locked box and labeled with child's name with instructions	Yes	No

Electrical Hazards
Are all electrical outlets covered?	Yes	No
Are there any exposed wires?	Yes	No
Are there any electrical cords hanging down or laying out? Cords that can trip or cause equipment to pulled down on children	Yes	No
Are all heaters/AC units blocked off?	Yes	No

Hygiene
Are Al1 hand sinks working properly?	Yes	No
Stocked with liquid hand soap, paper towels, water at 90°F-120°F?	Yes	No
Are children given enough time to wash hands?	Yes	No
Is hand washing encouraged and supported by staff?	Yes	No

Playground Hazards
Is the playground free from hazards? Hazards include broken glass, rodent or bug nesting, nails coming through playground structures, holes on playground. Anything that has a potential to cause injury is a hazard.	Yes	No
Do all gates and doors shut properly?	Yes	No
Is playground fence in good working order? No gaps more than 5 inches connected at top and bottom	Yes	No
Are all crawl spaces or openings under house blocked off?	Yes	No
