JACKSON, Tenn.(WATE) - 6 News learned two fatality reports from the Tennessee Occupational Safety and Health Administration mistakenly listed the location of the incidents as the Knoxville-based Gerdau steel plant, but the actual location was in Jackson, Tenn.

No accidents were reported prior to Thursday's at the Knoxville-based plant going back 10 years.

TOSHA said the computer system mismatched the establishment locations. The agency said it will correct the computer system but it will take a couple of weeks before the website reflects the corrections.

One incident report shows an employee was killed on the job at the Jackson steel mill location in February of 2008. The OSHA report shows the employee was climbing a 14-foot metal ladder and fell to his death in that incident.

Less than two months later in April of 2008, an OSHA report says a crane operator was standing on the crane landing waiting to enter the crane and fell 40 feet to the ground, also happening in Jackson. He died from the fall.

A Gerdau spokesperson alerted 6 News of the error on the TOSHA reports, which incorrectly listed Knoxville as the location. 6 News then confirmed the error with TOSHA.

Six employees were injured in Thursday's explosion at Gerdau's Knoxville location.

"The company is working with the proper authorities to determine the root cause of the incident," a spokesperson for Gerdau said.

Gerdau said five employees were treated for minor injuries and were released from the hospital. One is suffering serious injuries and is being treated at a regional burn center.
